Coumadin Clinic

Our goal is to optimize the safety and efficacy of anticoagulation therapy in patients taking Coumadin® (also known as Jantoven® or warfarin) through education and monitoring.

Offering 3 convenient locations and readily accessible clinical staff, the Coumadin Clinic of IRMC is the preferred warfarin management service for many patients and their physicians in the area. The clinic has been recognized as an Anticoagulation Center of Excellence (2015-2017) by The Anticoagulation Forum, a multidisciplinary non-profit organization that works to improve the quality of care for patients taking antithrombotic medications.  Our clinical pharmacists all have advanced training or certifications in the areas of anticoagulation and ambulatory care pharmacy. IRMC is also affiliated with several pharmacy schools, including the University of Florida and Ohio Northern University, and our pharmacists provide training and guidance to 4th year pharmacy students completing internships.

What is Coumadin®?

Coumadin®, or the generic called warfarin or Jantoven®, is a drug used to prevent blood clots from forming within the body. These clots may lead to stroke, pulmonary emboli, or deep vein thrombosis. This medication is an excellent drug for the prevention of clots; however, numerous drug-to-drug interactions, as well as drug-to-food interactions may intensify or nullify the effectiveness of warfarin. The administration of the drug must be monitored and managed for each patient in order to prevent complications, such as bleeding. Warfarin is often used for conditions such as atrial fibrillation, DVT (deep vein thrombosis), PE (pulmonary embolism) and for people with mechanical heart valves. Although several drugs are now available for some of these conditions, warfarin still remains the preferred agent in many patients.  


The process to begin care at the Coumadin Clinic starts with a referral by your physician that includes goals and the reason for the treatment. A copy of our referral form can be found here. Once we receive the referral from your doctor the Coumadin Clinic will contact you to schedule your initial appointment.

During your visit, your past medical history and diet will be reviewed. The pharmacist will also assess your medications, both prescription and over-the-counter, vitamins, herbals, and any other holistic products you use.

2Once your history is reviewed, a drop of your blood from a finger stick is tested using a point-of-care device called CoaguChek® XS Plus. This device measures the time needed for the blood to clot, also known as the International Normalized Ration (INR). Using this result and other patient-specific information, the clinic can make appropriate decisions regarding the management of your Coumadin®.

The pharmacist may adjust your dose of Coumadin® and review your therapy. A formal progress note is then developed and a copy of the visit is sent to your referring physician.
